  PROGRAM STATEMENT OAKHILL AFH 482 N Oakhill Ave. Janesville Wi, 53548 Oakhill Adult Family Home, licensed Adult Family Home, under Wisconsin Administrative Code HSS 88, to serve up to four individuals 18 years or older with developmental disabilities and advanced age. People with developmental disabilities are the primary client group in Oakhill AFH. It is located in Janesville, Wisconsin. Oakhill AFH has specifically chosen this group of individuals based on past experience. Matt Wilstein is experienced with this population in the day services environment. Matt Is also a former restaurant owner able to prepare and serve healthy delicious meals, prepared in accordance with health department guidelines for safe food handling. Matt Wilstein and Oakhill AFH has the capacity and training to provide physical and emotional care, and recognize and meet the individual needs of each resident. Matt has completed First Aid Training and CPR . He also has the experience, education, and training necessary to work with individuals with obsessive compulsive disorder; Multiple Sclerosis; Traumatic Brain Injury; Cerebral Palsy; Autism; ADHD; eating disorders and nutrition including Obesity; Diabetes (diabetic care/ insulin dependent – carbohydrate counting/sliding scale); Schizophrenia and Bipolar disorder; Borderline Personality Disorder. Oakhill Adult family Home strives to promote a family living environment, where individuals have the highest quality of life possible. Oakhill AFH is not currently wheelchair accessible only the addition of ramps at entry and exits to the home. There are plans to complete this upgrade in 2017. DESCRIPTION Oakhill AFH Is a ranch home with 2 private bedrooms and one semi-private bedroom for client residents. One shared full Bathroom with a bathtub. The Home has a full kitchen with a dining area and a spacious Living room. The home has a Golden doodle Diabetic alert service dog that is also working on becoming a therapy dog. Two affectionate cats are also members of this loving home. Oakhill AFH has a large grassy backyard with mature trees creating a comfortable homey environment. Oakhill AFH is located one block from a church and park. The bus route is 3 blocks away and some shopping is within walking distance. ADMISSION Individuals considered for admission to Oakhill AFH, must be programmatically compatible with each other. Similar likes, dislikes, and physical abilities are taken into consideration during the initial intake assessment. Oakhill AFH welcomes individuals who exhibit challenging behaviors. Initial assessment will be completed by the Home prior to admittance. However, the Individual Service Plan (ISP) will need to identify the areas of risk and serve as a reasonable plan to minimize Individual challenges. Individuals must be examined by a physician prior to admission and determined to be free of communicable disease. Documentation is required upon admission. Letters of guardianship, protective placement, or mental health commitments must be received upon admission. COMPATIBLE CLIENT GROUPS Oakhill Adult Family Home will be providing services to the Advance age and the developmentally disabled population. Only non-violent will be accepted. Both the AA and the DD population have similar needs (physically, emotionally and medically). This AFH will be providing services to the ambulatory. All residents and their case worker or representative and guardian will be interviewed and accessed; needs and desires of clients will be reviewed to ensure all residents are compatible. Past ISP and proposed ISP will be reviewed to determine that all the needs of each client are respected and encouraged. All aspects of all residents personality, needs, desire and wants will be reviewed to ensure all are compatible. GOALS The main objective of Oakhill Adult Family Home is to provide individuals with a safe, homey-like environment, in a family setting for those who do not have the capacity to live independently at home or out in the community. The home is designed to develop and maintain the skills and independence of the individual. This will be accomplished through attitudes of hopefulness, a structured, yet warm environment, individual treatment plans with realistic goals, and the opportunity for group living and social interaction. We provide daily support, teach and assistance promoting his or her health, well-being, self-esteem, independence and quality of life in the community. Vital to the program is an importance on community integration to the greatest extent possible for each resident. Each resident will have a goal oriented Individual Service Plan that will be discussed and reviewed with case managers, guardians, families, and others involved in their lives. SPECIFIC PROGRAM GOALS ? Recreational and enrichment activities that will promote the healthy development of the adult. ? Help the adult through the grieving and adjustment process that accompanies removal from their own home and placement. ? Monitor their mental health/physical and help the adult with their emotional needs. ? Provide transportation to activities, employment and appointments. ? Keep the guardian, placing agency, case managers, and family informed of changes, concerns, and progress of adults. ? Monitor the adult’s health by observing and documenting changes, and consult with medical providers when necessary. ? Schedule all medical appointments in a timely manner. Annual physicals, eye exams and dental exams to be completed. Records are kept of all medical appointments, reports, recommendations, and follow up care. ? Monitor medication reactions/seizure activity. Assist with medication administration daily and maintain accurate medication logs. ? Assisting and teaching personal care activities such as dressing, grooming, and bathing. ? Assisting and teaching daily living skills such as money management, meal planning and preparation, laundry, and household chores. ? Encouraging and helping residents in maintaining involvement with family members and other significant people in their lives. ? Accurately assessing and responding to the distinctive and changing needs of each adult. ? Provide opportunities to participate in community activities or events that he or she desires. STAFFING PATTERNS Oakhill Adult family Home has at least one staff on duty at all times. The ratio of staff to residents adequately meets the needs of each individual as defined in their assessments and individual service plans. SAFETY ISSUES Oakhill AFH will assist in teaching and supporting residents in development and understanding of areas where precautions may be necessary, such as emergency plans in case of fire or severe weather, and community safety. COMMUNITY RESOURCES AND IN HOME ACTIVTIES Oakhill AFH provides ample opportunity for all residents to participate in cultural, religious, political, social, and intellectual activities or events of his or her choice within the home and the community. The Home stresses community involvement through frequent attendance at activities and utilization of various resources in Janesville and surrounding areas. Some of the resources available in the community are museums, camping, The Capitol, churches, movie theatre’s, department stores, public and state parks, fishing, sporting events, zoos, botanical garden, parks –picnic rounds, restaurants, library, bowling, fitness center, fairs, etc.. Family-style recreational activities include playing cards and board games, listening to music, various arts and craft projects, viewing television and videos and reading books, newspapers or magazines, playing in house games. Participation in exercise programs, walking, and sports activities are also offered and encouraged.
