Attestation Form Possible Answer to Licensing Delay

Since Since the Bureau of Assisted Living has implemented the centralization of initial licensing and Change of Ownership, WALA has been hearing from providers the experience of longer delays in getting licensed. The BAL heard this as well through group meetings such as the Assisted Living Summit and the AL Forum and has developed proposals to speed up the process.


One of the many ideas was announced at the last AL Forum, held via Adobe Connect on January 12. At the provider breakout sessions each of the regulatory categories was able to get a glimpse at a draft attestation form. This attestation form would be the last item they would submit to the Bureau before their initial survey. Providers would attest that everything that was submitted to BAL is completed fully and accurately, adhering to regulatory standards. Many of  the items that were on this draft attestation form are also found on the Initial Licensure Checklist (link to CBRF Checklist). This attestation form will help reduce the time spent reviewing some of the  application details, thus speeding up the licensing process.


Since BAL will not be reviewing every aspect of application, they are exploring ways to ensure the truthfulness of the attestation. The Bureau may use measures such as citations or denial of the application if it is found at the initial visit, that the provider did not properly follow the regulatory guidelines for licensure. Denial of initial applications is rare; with Alfred Johnson, Director of the Bureau of Assisted Living, citing that roughly 2% of initial applications are denied. This newly developed process will be vetted again with the provider community in the next couple of months before finalization from BAL.
