The Wisconsin Assisted Living Association (WALA) is proud to announce a benefit for WALA members only. The WALA SFM Workers Compensation Insurance Program's goal is to provide valuable enhanced services to our provider membership at no additional cost.

First, You Need To Be A Member Of WALA

Please refer to the Membership page for more information on becoming a WALA member.

Second, Access The Insurance Company

M3 Insurance and SFM Mutual Insurance Company has put together this program to make access to Workers Compensation Insurance easier and more affordable for providers. Once you are a member of WALA you can call M3 and have the application sent to you.

The WALA Workers Compensation Insurance Plan Can Be Accessed A Couple Of Different Ways

Through the WALA members only access you will find a link to the application and rating system. If you do not have online access you can contact M3 directly and they will send you the application. You will need to complete the application, sign, and submit to the insurance agency with your payment and other documents requested in the application. The rating is easy and as we mentioned very affordable with NO minimum premium and the premium will be pro-rated based on when during the policy period you join the plan.
