Community Nurse - Job Description
Overview:
- Coordinate 24/7 healthcare services for assigned residents.
- Lead service planning, goal setting, and implementation of health care plans.
- Supervise House Managers and caregivers to meet resident needs.
- Provide clinical services and staff training.
- Oversee medication delegation and health care tasks per state regulations.
Key Responsibilities:
- Work with Executive Director to meet resident health needs and ensure proper care.
- Supervise and coordinate resident services, including care by external providers.
- Develop and update individual service plans, ensuring timely assessments and compliance.
- Monitor health, medication, and treatment administration; document results.
- Act as a resident advocate and ensure quality care.
- Investigate and resolve complaints; ensure regulatory compliance (HIPAA, OSHA, etc.).
- Administer first aid and evaluate health emergencies.
- Coordinate health care clinics and resident transitions (move-in/out).
- Supervise medication orders, therapeutic diets, and records maintenance.
- Identify training needs and conduct staff training.
- Ensure adherence to company culture and policies.
Qualifications:
- Registered Nurse (RN) with valid state license.
- Minimum 1 year in home health or geriatric nursing.
- Strong supervisory, communication, and training skills.
- Ability to perform physical tasks (lifting up to 50 lbs).
- Knowledge of HIPAA, State Regulations, and OSHA compliance.
- Willingness to work in a supportive, team-oriented environment.
Other Responsibilities:
- Participate in marketing, staff recruitment, and life enrichment activities.
- Ensure infection control procedures and quality standards are met.
- On-call duties and emergency care coordination as needed.
Job Type: Full-time
