Job Position Description:

We are currently seeking a Clinical Care Manager to join our dynamic team. This position plays a key role in ensuring the highest degree of quality care for our residents. You will be responsible for planning, organizing, and overseeing the overall operation of our care services, ensuring compliance with resident needs, government regulations, and our internal policies.

Essential Responsibilities:

• Oversee care coordination for all residents, including but not limited to:

  • Coordinate service plans

  • Activate appropriate resources

  • Communicate with physicians or pharmacies regarding medication changes or issues

  • Follow up on residents admitted to the hospital and complete necessary paperwork

• Conduct medical assessments for each new admission and any ongoing changes in residents' conditions

• Provide recommendations to the Executive Director regarding new admissions

• Manage medication administration, medication records, and documentation in the MARs (Medication Administration Records)

• Maintain resident documentation, including medical management and care plan records

• Lead and delegate nursing tasks to direct care staff in compliance with the Nurse Practice Act and regulatory requirements

• Continuously seek out and direct quality improvement activities focusing on wellness, clinical management, medication management, risk management, staff training, and safety/survey preparedness
